2) The following downloadable forms require Adobe Reader. To download the latest version of Adobe Reader, go to www.adobe.com before proceeding.
3) Print the LBC Development Bank application form.
4) For first time applicants, it is a general policy of the bank for the applicant’s personal appearance. Please bring the application form and requirements enlisted in the said form, together with 2 valid IDs & photocopies of IDs to your nearest LBC Development Bank branch.
5) Acceptable identification are:
PRIMARY IDs:
• Passport
• Driver’s License
• Laminated Company ID of reputable institution
• Laminated School ID of known institution
• License to carry Firearm
• Membership Card of known and reputable establishment or organization
• License to carry Firearm
• Membership Card of known and reputable establishment or organization
• Alien Certificate of Registration
SECONDARY IDs:
• SSS ID
• GSIS ID
• Tin Card
• Postal ID
• Voter’s ID
• Professional Regulations Commission ID (PRC)
6) Once the Application Form and document requirements are received by LBC Development Bank, you will be notified through mail regarding the status of your application.
